Welcome to the Provider Information area of our website. We are continuing to develop this area to assist both currently contracted providers with Region 3B and those who wish to inquire about an application to become a provider of services. You do not need a login or password to use this area and download documents.
For Current Providers – ONLY:
As previously announced, please find our FY 2012 edition of the Direct Purchase of Services Standards (DPOS) for Region 3B attached to this section. You will also see a memo that explains the revisions that are included as part of the new MI-Choice Waiver and Office of Services to Aging requirements, along with several items amended by Region 3B.
Annual Assurances for all DPOS, AFC/HFA Providers:
Please find attached the complete set of Assurances for FY 2012. Please read them over carefully and refer to the additional document with links for the legal notations to assist you in understanding the items for which you are signing. We are requesting ALL documents in the packet be signed and returned to our offices (with original signatures ONLY) by close of business, January 16, 2012. NO fax copies will be accepted. Even if you do NOT currently serve Calhoun County participants, we are asking that you sign this assurance anyway – in the event we may request your services.

Please look over our DPOS standards, located above, and review our requirements for the particular area of service you would like to offer. If you believe your agency would be able to provide this commitment, please fill out our New Provider Application form and e-mail it to our Contracts Manager, Penny Campbell at This e-mail address is being protected from spambots. Our Care Management group reviews applications for interested providers twice monthly and will contact you by phone or e-mail for additional information.
